You gotta remove the steel sleeve . Grease up the bush on the outside on the leading end , position as shown and do up the nuts to put a bit of pressure on it , enough to start distorting the bush , then shove a screwdriver (greased ) down the side of the bush and into the hole in the tca....the ide...

Another thing that can happen is the rear seal in the cylinder can fail . This can be awkward as it usually sucks air in and doesnt actually leak fluid . all the way from 200degC......so not really for road use ? I really cant remember seeing someone pull up to the lights with glowing red disc's, but anyhow Im done with some....one telling me that 2 pot solid disc's are as good a 4 pot vented disc's i mean fuck it!!, why not run drums all round :doh:
